A digital clock with an alarm, both in one tab. Set the time, leave the tab open, and a tone sounds when it arrives. It is deliberately simple: no accounts, no notifications, no permissions beyond letting the page play a sound.
Keep the tab open
The alarm runs entirely in this page, so it needs the tab to stay open — closing it cancels the alarm. Browsers also block audio until you have interacted with the page, which pressing the set button satisfies. For anything you genuinely cannot miss, use a device alarm as well.
